By submitting this form I give permission for my child(ren) to take part in the activities of the above named group at the Ernest Street premises or at pre planned locations. I understand that while involved he/she/they will be under the control and care of the group leader and or other adults who have been approved by the church leadership and that, while the staff in charge of the group will take all reasonable care of the children/young people, they cannot be necessarily held responsible for any loss, damage or injury suffered by my child during, or as a result of any activity within the group.

By submitting this form I give permission for my child, in an emergency and/or if I am not contactable, to receive necessary hospital or dental treatment including an anaesthetic if necesssary.

• understand they will use this information to get in contact in an emergency, to inform me about events and activities that are happening at the church.

(Ernest Street will remove these records if I have not been to any events for a 12 month period.)
